The Key Role of Urine Microalbumin(ALB) Testing in Early Detection of Kidney Failure

Urine Microalbumin(ALB) testing, or the urine albumin-to-creatinine ratio (UACR), is an important indicator for early detection of kidney damage. It can indicate kidney damage even before significant decline in renal function (e.g., serum creatinine and estimated glomerular filtration rate (eGFR) remain normal). It is particularly crucial for early screening of kidney disease in high-risk individuals, such as those with diabetes and hypertension.

1. Why can ALB testing detect renal failure early?

Healthy individuals’ urine contains only minimal amounts of albumin. However, when the glomerular filtration membrane is damaged, albumin leaks into the urine. Microalbuminuria (UACR 30-300 mg/g) is an early sign of diabetic nephropathy and hypertensive nephropathy, appearing 5-10 years before elevated serum creatinine.   • Macroalbuminuria (UACR >300 mg/g): Suggests significant kidney damage. Immediate medical attention is required to identify the cause and initiate intensive treatment.

4. Important Testing Considerations

  • Repeat testing is necessary (temporary increases may occur due to exercise, infection, etc.).
  • Combining eGFR, serum creatinine, and other indicators to comprehensively assess renal function.
